Description:

Linagliptin + Metformin Hydrochloride is a combination medication containing Linagliptin (a DPP-4 inhibitor) and Metformin (a Biguanide), belonging to the anti-diabetic class. Pharmacology

Linagliptin is prescribed to assist individuals with type 2 diabetes in regulating their blood sugar levels. It inhibits DPP-4 (dipeptidyl peptidase-4), an enzyme responsible for breaking down GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide), which are incretin hormones. By inhibiting DPP-4, Linagliptin enhances the release of insulin from pancreatic beta cells in response to glucose levels. Additionally, it reduces the production of glucagon from pancreatic alpha cells, leading to higher active incretin hormone concentrations.

Metformin Hydrochloride is a medication from the biguanide class used to treat type 2 diabetes. It affects both baseline and postprandial (after-meal) plasma glucose levels.   • For patients switching from separate linagliptin and metformin therapy: Adlinameg should begin with the same dose of linagliptin and metformin that the patient was previously taking.
  • For patients inadequately controlled on dual therapy of metformin and sulphonylurea: Adlinameg should comprise linagliptin at 2.5 mg twice daily (5 mg total daily dose) and a metformin dose comparable to the ongoing one. In such cases, the sulphonylurea dose might need adjustment to lower the risk of hypoglycemia.
  • For patients inadequately controlled on dual therapy of insulin and metformin: Adlinameg should include linagliptin at 2.5 mg twice daily (5 mg total daily dose) and a metformin dose similar to the ongoing one. Adjusting the insulin dose may be necessary to decrease the risk of hypoglycemia.
  • Elderly patients: Caution should be exercised as age increases due to metformin being excreted by the kidneys. Renal function monitoring is crucial, particularly for patients over 80 years old.
  • Renal impairment: Adlinameg should not be used in patients with moderate or severe renal impairment (creatinine clearance < 60 ml/min) due to metformin.
  • Hepatic impairment: Adlinameg is not recommended for patients with hepatic impairment due to metformin. Limited clinical experience exists for hepatically impaired patients.
  • Pediatric population: The safety and efficacy of Adlinameg have not been established for children and adolescents aged 0 to 18 years.
